Annual Report Forms

These forms are not available as blank forms and must be filed on the preprinted forms provided by the Division of Business Services. The forms are prepared and mailed to each active company on an annual basis. Requests for replacement annual report forms may be made to the Division via e-mail, by telephone, in writing, or by submitting an online Replacement Annual Report Request.
